Your NDIS Plan

When you become a participant in the NDIS, you receive funding that is tailored to your unique needs. This funding is based on what is considered reasonable and necessary to support your goals, health, and well-being.

Eligibility and Application

To access NDIS funding, you must first confirm your eligibility. The NDIS is available to Australian residents who have a permanent and significant disability. If eligible, you’ll need to gather necessary documentation and complete required forms. Professional verification of your disability may be required as part of the application process.

Planning Meeting and Assessments

Once your eligibility is confirmed, you’ll participate in a planning meeting. This is where you discuss your current supports, goals, and the types of services you need. The information gathered during this meeting helps to develop your NDIS support plan.

Receiving Your Approved Plan

After your planning meeting, the NDIS will approve your plan, which includes funding for the supports deemed reasonable and necessary. You’ll receive an ‘access decision’ letter, and then your plan will be created. You can receive your plan in various accessible formats if needed3.

Using Your Plan with Recognise Support Services

With your approved NDIS plan, you can start using your funding to access services provided by Recognise Support Services. This includes support coordination, recovery coaching, and other services that align with your plan goals. Recognise Support Services will work with you to ensure the services provided are within your plan’s budget and meet your needs.

Plan Management

You have the option to self-manage your funding, have it plan-managed, or NDIA-managed. Recognise Support Services can assist you regardless of how your plan is managed, ensuring you get the most out of your NDIS funding.

Changing Your Plan

If your needs change, or if you find that your supports are not working as expected, you can request a plan reassessment. This ensures that your supports continue to align with your goals and that you have the funding necessary to achieve them.
